Dear Parent/Guardian,
Every year the Van Buren/Cass District Health Department performs Hearing and Vision Screenings on students from Preschool to 9th grade.
If you do not want your child to participate in the screenings, please complete this form and return to your child’s school office for their records. This needs to be completed each year for non-participation.
If your child is screened with results being not in the “normal” range, you will be notified by letter. If you receive a referral letter, it is recommended that you take your child to a doctor for evaluation.
If you have any questions regarding this matter, please contact the Van Buren/Cass District Health Department Hearing & Vision Program.
